For those with heart disease, our team can coordinate a program following
your physician's guidelines. The Garon team offer four phases of rehabilitation.
Phase I begins while you are still in the hospital, with education about
heart disease and instruction for recovering at home.
Phase II and Phase III offer a variety of strength building and aerobic
exercise, building in intensity as you recover.
Phase IV helps you plan and maintain a healthy exercise program.
Phase One
- Available for hospitalized patients following any heart procedure
or complication from heart disease
- Begins with a referral from the patient's attending physician
- Offers group counseling with patients and their families
- Contains specifically designed educational sessions focusing on understanding
how the heart works, the effects of illness or surgery and home instructions
- Facilitates referral to a Phase II cardiac rehab program
Phase Two
- Hospital-based outpatient program that follows a referral from the
participant's physician and completion of an exercise stress test
- Also available to patients who were not hospitalized at Jewish Hospital
- Utilizes a variety of aerobic exercises, strength training and flexibility
exercises to condition and maintain cardiovascular endurance and functional
ability
- Involves three one-hour exercise sessions held each week for up to
10-12 weeks
- Includes educational classes with registered nurses, registered dietitians
and exercise physiologists to learn risk factor modification and stress
management
- Provides a book and video library with information on various subjects
related to heart disease and heart healthy living
- Includes a stress management seminar for participants and their family
members
Phase Three
- Serves as a bridge for individuals who have completed a Phase II
program and need closer supervision before progressing to a Phase IV
maintenance program
- Requires a referral from the participant's physician and completion
of an exercise stress test
- Includes medically supervised exercise sessions with periodic telemetry
monitoring
- Utilizes a goal-oriented, independent exercise format to assist participants
in maintaining their commitment to a healthy lifestyle
- Provides a book and video library, classes, seminars and individual
counseling available to participants and their families
Phase Four
- Offers a maintenance program for individuals who have completed a
Phase II program or have been diagnosed with heart disease and are interested
in exercising in a supervised setting
- Requires a referral from the participant's physician and completion
of an exercise stress test
- Includes medically supervised exercise sessions with periodic telemetry
monitoring
- Utilizes a goal-oriented, independent exercise format focusing on
maintenance of health through exercise and risk factor modification
- Provides a book and video library, classes, seminars, special events,
fitness testing and individual counseling to participants and their
families
- Incorporates individualized strength training and flexibility programs
for all ages and all levels of fitness
