Claude C. Hazlett, M.D.

Claude C. Hazlett, M.D., a native of Ashland, Ky., has been with Haller, Hazlett & Adams since 1965. He received his bachelor of science from the University of Kentucky and his medical degree from Wake Forest University, Bowman Gray School of Medicine. Dr. Hazlett completed his postgraduate training in Family Practice at Greenville General Hospital in Greenville, South Carolina.

Dr. Hazlett is a member of the Jefferson County Medical Society, Kentucky Medical Association, and the American Academy of Family Physicians. He is married and has one daughter.
