Interdisciplinary Team of Experts

Frazier Rehab Institute’s Stroke Rehab Program is accredited by the Commission on the Accreditation for Rehab Facilities (CARF). The stroke rehab team includes many professionals who are highly qualified to lead patients through the stroke recovery process. The team meets at least weekly to address progress and continue planning for discharge. The patient and family are considered vital members of the team. The following professionals may be involved in a patient’s care:

  • Board-Certified Physiatrist (specialist in rehab medicine)
  • Nurse
  • Case Manager
  • Registered Dietitian
  • Physical Therapist
  • Occupational Therapist
  • Speech-Language Pathologist
  • Therapeutic Recreation Specialist
  • Pulmonary Rehab Clinician
  • Psychology Professional
  • Neuropsychologist

Our case managers act as the team coordinator, linking the family, patient and rehab team. The case manager works with the insurance companies to report progress and to advocate for the continuing stay at Frazier while progress is occurring. In addition to coordinating and scheduling family education and team conferences, the case manager supports the patient and family in discharge planning by identifying services, support, equipment and community resources that will be needed after inpatient rehab.

The team can also assist in addressing spiritual needs by connecting the patient and family with available religious services provided through the medical center. If preferred, an individual’s personal religious ministers may be called in for spiritual support.
