Outpatient Rehab Recovery from a brain injury is a long-term process. While most patients are discharged home after only a few weeks of inpatient rehab, ongoing services will be needed to continue the recovery process. The rehab team will make recommendations for continued therapy, which may include outpatient therapy with Frazier’s NeuroRehab Program or at any of Frazier’s outpatient locations, or home health therapy through VNA Nazareth Home Care or another home care organization. NeuroRehab Program Frazier’s NeuroRehab Program is an intensive program that works with individuals with acquired brain injury and other neurological conditions. Patients participate in individual and group therapies throughout a structured day to acquire and build on functional skills to assist them at home and in the community. The NeuroRehab Program provides a comprehensive team of professionals including speech-language pathologists, occupational therapists, physical therapists and psychologists. The team approach offered to the brain injured outpatient in the NeuroRehab Program is individualized to decrease the level of disability and the need for assistance, while increasing productivity in the home, workplace and community. Services offered at the NeuroRehab Program include:
Skilled Nursing and Residential Care Some patients who continue to require 24-hour nursing care and a less intense level of therapy may be discharged to a skilled nursing facility. Other patients may need a more intensive, longer term residential treatment program to achieve more progress before being able to return to the home setting. We can make appropriate referrals for skilled nursing facility placements and specialized residential brain injury programs if necessary. Additional Information
