Patient Evaluation & Care A UNOS certified cardiologist with expertise in congestive heart failure and cardiac transplantation directs the evaluation and care of patients referred to the Heart Failure and Cardiac Transplant Center. The Heart Transplant Team coordinates immuno-suppressive management of the patient during heart transplant and after discharge. The team consults on the patient's status with the referring physician throughout the transplant process. Patients referred to our Transplant Center undergo a complete medical assessment specific to their needs. This assessment may include:
Patients are also screened for anticipated compliance, support system availability, and emotional stability. Additionally, financial coverage for the transplant and follow-up care is confirmed. Transplant nurse coordinators provide continuity throughout the entire transplantation process, from referral to follow-up aftercare. Transplant social workers and financial counselors assist the patients and their families with temporary lodging accommodations, insurance verification, and other pertinent matters.
