The
Jewish Hospital Diabetes Management Program helps those who have been
diagnosed with Type 1 and Type 2 diabetes control their disease so they
can lead an active and healthy lifestyle. Patients and their families
work individually with nurses and dietitians who are certified diabetes
educators to develop a personalized program to fit the patients
lifestyle.
Diabetes Self-Management Education Program includes:
- Private consultations with registered nurses and registered dietitians
to discuss treatment, self-care skills, and dietary habits and to set
goals for managing a healthy lifestyle
- One-day seminar that gives patients and families helpful information
to support lifestyle changes
- Updates on patients progress sent to patients physician
- Follow-up with diabetes educators
